You can enable EEO and OFCCP Surveys from your Compliance settings if you go to Profile - App settings - Compliance and open the EEO/OFCCP tab.

After you enable the surveys, please note that they will only appear on application forms for jobs that have the country set as the United States.

On the application forms, the surveys will look like this.

NOTE: All answers will be collected and saved anonymously so they will not be connected to any of the candidates, but you will be able to keep track of them, in general, from your EEO Analytics reports.
